This is the survey used in the study described in the following paper:

1. Start

This survey has questions about how you collect and explore personal information. Personal information is information about you and your behavior that is recorded over time, e.g., your spending habits, your energy usage, your weight, your health records, etc.

After completing the survey, you will be entered into the raffle for a $25 Amazon gift certificate. Please leave your correct email address at the end of this survey, so that we can contact you if your name is randomly picked.
